Medical In and Out Processing

All of this information is located in a member's vMPF checklist when they click the checklist item for "Medical Out-processing," though it will be a condensed version as it is limited by characters.
A minimum of 45 days is required to review medical information and process paperwork and vMPF checklist. Members are asked to not wait until the day prior to their final out appointment to provide the clinic with the paperwork, clinic officials will not be able to complete it then. If members have extenuating circumstances, please call the clinic at the numbers listed below as soon as notification is received of the circumstance.
All Air Force members out-processing for a Permanent Change of Station (PCS) move, retirement or separation (even Geographically Separated Units, or GSUs), must complete and submit a Medical In and Out-Processing Questionnaire and Family Member Disclosure Form to the Medical Management team no later than 45 days prior to a PCS date/move. Please submit the forms via MiCare Secure Messaging (SM)/Relay Health account or hand carry forms to the clinic front desk. Completed forms cannot be sent via email to either encrypted or unencrypted organization boxes, in accordance with HIPAA regulations.

All Air Force members in-processing to receive medical care at Hanscom and Air Force GSU members will complete the same form as above. If service members are located at a GSU, and do not attend the Hanscom Base Newcomer's briefing, please download, complete and return the form via MiCare Secure Messaging (SM) account or hand carry forms to clinic front desk. Completed forms cannot be sent via email to either encrypted or unencrypted org boxes, in accordance with HIPAA regulations.
- To register for MiCARE/Relay Health, email email@example.com for a registration form or complete one at the front desk.
- If a GSU member and already have MiCare/Relay Health, email firstname.lastname@example.org so clinic officials can add a Hanscom provider to an existing account.
Overseas PCSs must complete a Medical Clearance Memorandum and submit it to MPS Relocation Counselor through MyPers at https://mypers.af.mil/app/answers/detail/a_id/13461/kw/13461.

Other military service members (Army, Coast Guard, Marine, Navy) are asked to follow their specific service instructions and if any paperwork is required for the MDS to complete, members will need to provide that paperwork to the clinic via MiCare/Relay Health or hand carry the form to the clinic front desk. Please follow the instructions in the Medical Out-processing handout and disregard the DHRAs portion.
