Wills and Health Care POAs
Have your will prepared, review it with an attorney and sign it at a will signing ceremony--all in one day. By appointment only, Tuesdays from 9 to 11 a.m., and Thursdays from 1 to 3 p.m. In order to schedule a will appointment you must first complete a Will Worksheet and, if your spouse will also be obtaining a will, a Dual Representation Authorization Form.
To do so, visit the Legal Assistance website to complete any of these documents. Worksheets can be found under the "Legal Worksheets" tab of the Legal Assistance website. Remember to give us your control number when scheduling your appointment. Please double check your spelling and information as we use exactly what you provide our office.
Powers of Attorney
To draft a power of attorney, please visit the Legal Assistance website prior to arriving to our office. Under the "Legal Worksheets" tab, choose the type of power of attorney you would like and fill in the relevant information. Then bring in the control number that was provided to you. This will expedite your time in our office. If you prefer, you can arrive to our office without visiting the website, but this will add time to the process.
